This article covers how to connect your JotForm account to FXMedSupport so patient-submitted forms are automatically imported into Cerbo.

The JotForms integration allows you to connect your JotForm account to FXMedSupport using an API key, so that forms submitted by patients are automatically imported into Cerbo as documents. You can configure which forms trigger on patient registration, enable auto-import, and manually pull in submissions at any time from the Import tab.

• Manually importing submissions for any specific form on demand
• Optionally deleting the original JotForm submission entry after import
• Reviewing imported form submissions directly in the Cerbo Patient Portal Queue

Before You Begin

• You must have an active JotForm account (Starter or higher)
• FXMedSupport must be installed and connected to Cerbo
• Your JotForm forms must already be built and published before setup
• You will need access to your JotForm account settings to generate an API key

Step 1: Open the FXMedSupport Widget

1. On the right side of the Cerbo screen, click the FXMedSupport circular icon to open the widget.

Step 2: Navigate to the JotForms App

1. At the top of the FXMedSupport widget, click Available Apps.
2. Scroll down to the Form Builder section.
3. Click JotForms to open the app.
4. The app will open on the Settings tab by default.

ℹ️ The JotForms app is listed under the Form Builder category in the Available Apps screen. You will see two tabs: Settings and Import.

Step 3: Generate Your JotForm API Key

You will need to complete these steps inside your JotForm account before returning to FXMedSupport.

1. Open a new browser tab and go to jotform.com.
2. Log in to your JotForm account.
3. Click your profile icon in the top-right corner of the screen.
4. In the dropdown menu, click Settings.
5. In the left sidebar of the Settings page, click API.
6. You will see the API Keys section. If you already have an existing API key listed, you may copy it and skip to Step 4.
7. If no key exists, click the green Create New Key button.
8. A new API key will be generated and appear in the list with Full Access permissions.
9. Copy the API key value — you will paste it into FXMedSupport in the next step.

💡 Tip: You can create and delete API keys in JotForm at any time. If you ever need to reset access, delete the existing key and click Create New Key to generate a fresh one.

⚠️ Warning: Keep your JotForm API key private. Anyone with this key can access your JotForm account data. Do not share it publicly or store it in unsecured locations.

Step 4: Paste the API Key into FXMedSupport

Return to the FXMedSupport widget in Cerbo.

1. In the JotForms app, confirm you are on the Settings tab.
2. In the Account Type dropdown, select Standard.
3. Click the API Key field and paste the API key you copied from JotForm.
4. Click the Refresh Forms button in the top-right corner to sync your forms. Your JotForm forms will now appear in the Forms and Auto Import Forms fields.

ℹ️ If your forms do not appear after pasting the API key, click Refresh Forms again. Make sure the API key was copied completely without any extra spaces.

Step 5: Configure the Settings

Once your forms are loaded, configure the following settings:

1. Forms — Click inside the Forms field and select which forms should be connected to FXMedSupport. Selected forms will appear as tags (e.g., Form2, First Form). These are the forms available for import.

2. Import On Registration — Check this box if you want patients to be prompted to fill out one or more forms when they register in Cerbo. When enabled, the selected forms will be assigned to new patients automatically upon registration.

3. Enable Auto Import — Check this box to enable automatic importing of form submissions as they come in. When enabled, a new Auto Import Forms field will appear.

4. Auto Import Forms — Click inside this field and select which forms should be auto-imported. These forms will be pulled into Cerbo automatically whenever a patient submits them.

5. Delete Origin Submission Entry — Check this box only if you want the original JotForm submission entry to be deleted after it is imported into Cerbo. Leave this unchecked if you want to keep the original submission in JotForm as well.

6. Once all settings are configured, click the blue Save Changes button.

⚠️ Warning: Enabling Delete Origin Submission Entry will permanently remove the form submission from your JotForm account after it is imported. Only enable this if you are certain you do not need the data retained in JotForm.

Step 6: Manually Import Form Submissions

Use the Import tab to manually pull in submissions for any form at any time.

1. Click the Import tab at the top of the JotForms app.
2. In the Forms dropdown, select the specific form whose submissions you want to import.
3. Click the blue Import button.
4. The app will process the import. Once complete, a confirmation message will indicate the data was synced successfully.
5. The imported form submissions will now appear in the Patient Portal Queue in Cerbo.

💡 Tip: Use the Import tab to manually sync older submissions or to test your integration after initial setup. This is especially useful after first connecting your JotForm account.

Step 7: Review Imported Forms in the Patient Portal Queue

Once a form is imported, it will appear in the Cerbo Patient Portal Queue for review.

1. In Cerbo, look at the Patient Portal Queue on the right side of the screen (under Pt Portal Queue — Unassigned).
2. Locate the patient entry that shows the submitted form (e.g., 'Coffffe Smith would like to add a form/questionnaire').
3. Click the green Save Document and Review button next to the patient's entry.
4. A review modal will open displaying the form content submitted by the patient.
5. On the right panel of the review modal, complete the following fields as needed:
• Title — the document title is auto-filled (e.g., First Form added 04/24/2026)
• Type — select the document type (e.g., Document Type: Forms)
• Subfolder — select a subfolder if applicable
• Results — set a result status (e.g., Normal)
• Follow Up — assign a follow-up task if needed
• Notify Patient — choose whether to notify the patient by portal or email
• Notes / Key Results — add any notes (a default note 'Submitted via portal' may be pre-filled)
6. Check or uncheck Flag this as Key Document, Add to Past Med History?, and Show in Pt's Online Portal? as appropriate.
7. Click Save (unreviewed) to save the document for later review, or click Save (filed/closed) to mark it as complete.

ℹ️ After saving, the form document will be filed under the patient's Cerbo record. You can access it at any time from the patient's chart in Cerbo.

Summary

Open FXMedSupport and navigate to Available Apps > Form Builder > JotForms
In JotForm, go to your account Settings > API and copy your API key (create one if needed).
Paste the API key into the API Key field in the JotForms Settings tab in FXMedSupport.
Set the Account Type to Standard and click Refresh Forms to load your available forms.
Select which forms to connect in the Forms field.
Enable Import On Registration to assign forms to new patients at registration.
Enable Auto Import and select forms in Auto Import Forms for automatic syncing.
Optionally enable Delete Origin Submission Entry to remove submissions from JotForm after import.
Click Save Changes to save your configuration.
Use the Import tab to manually import submissions for any selected form.
Review imported form submissions in the Cerbo Patient Portal Queue via Save Document and Review.

Troubleshooting

Forms are not appearing in the Forms field after entering the API key → Click the Refresh Forms button in the top-right corner of the Settings tab. If forms still do not appear, verify the API key was copied correctly from JotForm with no missing characters or extra spaces.

API key is not being accepted → Confirm that the API key in JotForm has Full Access permissions. Go to JotForm Settings > API, delete the existing key, click Create New Key, and copy the new key with Full Access selected.

Imported form submissions are not appearing in the Cerbo Patient Portal Queue → Go to the Import tab, select the form, and click Import again. If the queue still does not update, refresh the Cerbo page and check the Patient Portal Queue.

Import On Registration is enabled but patients are not seeing the form → Confirm that the correct forms are selected in the Forms field on the Settings tab. Click Save Changes again to ensure the configuration was saved correctly.

Deleted JotForm submission entries by mistake → The Delete Origin Submission Entry option permanently removes entries from JotForm after import. This cannot be undone. Uncheck this option going forward to retain future submissions in JotForm.
